Subclinical Hypothyroidism; Natural History, Long-Term Clinical Effects and Treatment

چکیده
Subclinical hypothyroidism, defined as a mild degree of thyroid dysfunction biochemically, is a common clinical disease. It is marked by elevated serum concentrations of thyroid stim‐ ulating hormone (TSH) and normal serum concentrations of free thyroxine (fT4) and triiodo‐ thyronine (T3). Subclinical hypothyroidism can be divided into two categories, depending on the magnitude of the increase in serum TSH, with concentrations of 4.5-10 mU/L consid‐ ered mild disease and TSH >10 mU/L considered severe disease. However, the definition and clinical significance of subclinical hypothyroidism are confounded by controversies over the exact upper limit of the reference range for se um TSH.

Subclinical hypothyroidism is defined as a serum thyroid stimulating hormone (TSH) concentration above the upper limit of its reference range plus a serum free thyroxine (FT4) concentration within its reference range. There are several controversies regarding the effect of subclinical hypothyroidism on the risk of heart failure, cardiovascular disease risk, cholesterol metabolism and mortality,...

Subclinical thyroid dysfunction (STD) represents a condition of slight thyroid hormone excess or deficiency, which may be associated with important adverse effects. This review will focus on the natural history, diagnosis and management of subclinical thyroid dysfunction. Since STD is only detected as a thyroid stimulating hormone (TSH) abnormality, it is essential to exclude transient causes o...

Subclinical hypothyroidism (SH) is a quite common disorder in the pediatric age group. The aim of this paper is to present a review of the studies investigating the natural course of SH and the effects of replacement therapy with levothyroxine in childhood. We systematically searched PubMed, Cochrane, and EMBASE (1990 to 2012) and identified 14 articles suitable to be included. SH is a benign p...

BACKGROUND Subclinical hypothyroidism is associated with a number of cardiovascular risk factors, yet only limited data exist on long-term outcome of levothyroxine treatment of this condition with respect to hard end-points. The aim of this retrospective cohort study was to determine effects of levothyroxine treatment on myocardial infarction (MI), cardiovascular death and all-cause mortality, ...
متن کاملNatural history of subclinical hypothyroidism in children and adolescents
Diagnosis of subclinical hypothyroidism (SH) is based on interpretation of biochemical tests in the absence of the evident clinical symptoms. Mildly elevated TSH with normal fT4 are common in adults and the prevalence of this finding is reported to be 1-10% of general population, being higher in the elderly. In pediatric population its prevalence is lower than 2%. Moreover in about 60% of subje...
